RBI Care, established in 2012, is an adult family home (AFH) located in Sparta, Wisconsin for individuals 18 years of age and older with a variety of client group such as: Alzheimer's, dementia, DD, mental illness and advanced age just to name a few.

RBI Caring Hearts, established in 2015, is a community based residential facility (CBRF) located in Bangor, Wisconsin for individuals 18 years of age and older with a client group such as: Alzheimer's, dementia, DD, mental illness and advanced age just to name a few.

Residents Receive:

• Three meals per day, with in-between meal nourishment

• Assistance with personal hygiene and grooming

• Medication monitoring

• Leisure time activities

• Community and local activities

• Transportation to medical appointments

• Health monitoring

• Daily housekeeping and laundry assistance

• RN delegation on-site
